Job Title: Social Worker

Tasks and Responsibilities:

Assessment and Counseling: Conduct comprehensive assessments to determine the psychosocial needs of patients and develop tailored care plans. Provide counseling and support to individuals and their families.

Advocacy: Act as an advocate for patients and their families, ensuring their rights and needs are met within the healthcare system.

Resource Coordination: Collaborate with healthcare professionals and community agencies to coordinate necessary resources and services for patients.

Crisis Intervention: Provide crisis intervention and emotional support during difficult times.

Documentation: Maintain accurate and up-to-date records of assessments, interventions, and progress.

Education: Educate patients and families about available resources, coping strategies, and community support.

Requirements:

• Knowledge to excel in this role, you should have the following qualifications:
  

• Bachelor's or Master's degree in Social Work (BSW or MSW)

• Proven experience as a Social Worker in a healthcare or related setting

• Strong interpersonal and communication skills

• Empathy and a compassionate approach to patient care 

  of relevant laws and regulations governing social work practice

• Ability to work collaboratively in a multidisciplinary team

Certification Requirements:

Certified Social Worker (CSW) or Licensed Clinical Social Worker (LCSW) preferred

State License Requirements:

• Michigan

Additional License Requirements:

• LPC, LMSW, LLP, or the respective limited license
