Responsibilities include relaying visual and environmental information, offering travel assistance, and ensuring safe community access through human guide support and transportation. The position also involves case management for low-income individuals and families, including client intake, assessment, advocacy, referrals, and maintaining accurate documentation. Ensures all services align with program goals and contractual requirements. Position may from time to time be stressful and require a high demand of performance.
