General Description of Position

Under the general direction and supervision of the Dean of Students, Milford Campus, with on-campus support from the Director of the Counseling Assistance Program for Students, the Student Care Case Manager provides support and care to students experiencing varying degrees of need and/or distress, as well as providing holistic, student-centered case management services to students experiencing complex personal, mental health, academic, financial, housing, or social challenges.

The Student Care Case Manager is readily accessible to students, staff, faculty, and family members (when applicable) who are concerned about a distressed student. The Student Care Case Manager works closely with key offices to identify and intervene as early as possible with students potentially experiencing basic need insecurities, distress, or other challenges who would benefit from advocacy and support services.

The full-time regular position requires exceptional communication skills to address behavioral concerns and support students in distress. Responsibilities include assessing individual student needs, developing and implementing tailored intervention plans, coordinating multidisciplinary responses, monitoring progress, and navigating complex legal, ethical, and institutional considerations. The position also collaborates with campus and community partners, maintains accurate documentation, evaluates program outcomes, and contributes to student well-being initiatives.

Essential Functions

Assistance & Support for Students

  • Provide direct outreach and case management services to students referred to the CARE Team, prioritizing high-risk and complex student cases.
  • Conduct assessments of student strengths, assets, risks, and barriers to determine appropriate intervention strategies and support resources
  • Develop, monitor, and evaluate intervention plans and progress for referred students, working collaboratively with campus partners to support student well-being and safety
  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and student-centered.
  • Serve as a liaison between students, providers, faculty/staff, & families
  • Provide direct intervention services to students experiencing emotional, interpersonal, or physical distress
  • At the direction of the Director of the Counseling Assistance Program for Students, provide consultation to faculty or staff responding to students experiencing distress
  • Communicate case updates, action plans, and intervention outcomes to CARE Team members and relevant campus stakeholders while maintaining appropriate confidentiality.
  • Maintain detailed documentation of CARE Team activities, case interventions, and outcomes within institutional case management systems.
  • Provide support to the SCC CAPS program by serving as a resource during campus crises or emergencies by providing student support following campus incidents or tragedies.

Program Development & Evaluation

  • In collaboration with the Dean of Students-Milford, promote awareness of CARE Team services and student support resources through outreach initiatives and campus engagement activities.
  • Participate in campus-wide committees and working groups as directed that address student well-being, safety, and institutional risk management.
  • Support the activities, programs, and services associated with the functions of the Student Success division.
  • In collaboration with the Dean of Students-Milford, engage in ongoing assessment and evaluation of CARE Team needs and recommend modifications to meet identified benchmarks.

Marginal Functions

  • May assist with college public relations and promotional activities.
  • May assist with new student activities, including New Student Welcome.
  • Works directly with the Director of Student Engagement, Director of Counseling Assistance program, and Residence Life staff to assist students.
  • Perform other related duties within the division/program as assigned.
  • May be required to perform associated duties, functions, or assignments in other divisions/programs/areas as required.
  • Perform other College functions and duties as assigned.
  • Must be able to work varied days, hours, shifts, locations, and campuses as required. Work schedules (hours/days/work location) are scheduled by Southeast Community College based on the needs of the College. Work hours, shifts, days, and work locations may vary depending on the needs of Southeast Community College and are subject to modification. Emergency or scheduled special activities may require hours outside of the regular workweek.

Required Knowledge, Skills and Abilities

  • Broad knowledge of student development, mental health care, and appropriate services/supports.
  • Ability to effectively communicate both in writing and orally to a diverse group of students, staff, and the public.
  • Strong communication, collaboration, conflict resolution, and advocacy skills, and the ability to work effectively with diverse student populations
  • Knowledge of post-secondary education, admissions, registration, and financial aid processing/procedures.
  • Understanding of higher education case management, HECMA/CAS standards, and FERPA regulations
  • High degree of personal/professional integrity, respect for privacy and confidentiality, and a strong sense of ethics.
  • Skill and ability to initiate, plan, implement, monitor, and evaluate plans of care.
  • Knowledge of student support functions and student development.
  • Skill and ability to promote the college and programs offered by the college.
  • Knowledge of student support services and delivery of these services.
  • Ability to relate effectively and work collaboratively with students and institutional personnel with varied missions, goals, and objectives.
  • Skill and experience in maintaining accurate documentation or case progress, evaluating progress, compiling data, and preparing reports.
  • Knowledge and ability with data processing (word processing, spreadsheets, databases, etc.).
  • Ability to handle multiple tasks, changing priorities, and perform tasks with a high degree of accuracy and attention to detail.
  • Ability to communicate effectively in writing, in person, and on the phone.
  • Ability to use basic computer skills.
  • Ability to lift and carry up to fifty (50) pounds for short distances and move tables, chairs, and media equipment when necessary.
  • The individual must possess the above skills and abilities or be able to explain and demonstrate that the individual can perform the essential functions of the job, with or without reasonable accommodation, using some other combination of skills and abilities.

Minimum Qualifications

  • Bachelor's degree in Social Work, Counseling, Psychology, Human Services, Student Affairs, Higher Education, Behavioral Science, or a closely related field.
  • Two (2) or more years of professional experience in de-escalation, behavior intervention, and/or case management.

Desired Qualifications

  • Experience in de-escalation, behavior intervention, and/or case management with college students or emerging adults.
  • Experience in prevention, psycho-educational, and educational outreach and programming for students, faculty, staff, and parents/family members.
  • Experience working in an environment requiring significant collaboration, including bridge-building with different groups and stakeholders.
  • Experience initiating and coordinating services for individuals and connecting them to comprehensive programs to help them succeed, as well as advocating on their behalf in extenuating circumstances and assisting them in understanding and accessing resources available to them.

Salary

$51,566 per year

Benefits

SCC BEN Dollars - Eligible Employees: Full-time status. Regular employees with an FTE of 0.75 or greater will be eligible for SCC Ben Dollars. All employees will be required to elect a health insurance option. If the cost of insurance coverage selected by an employee exceeds the SCC Ben Dollars available, all additional costs will be withheld from the employee's paycheck. Any SCC Ben Dollars that the employee does not elect to use for the purchase of dependent health and dental insurance will be paid to the employee in cash at $0.93 per $1.00 benefit dollar. All amounts paid are subject to withholding for income and employment taxes, but not subject to the College's contribution toward the Retirement Savings Plan - Group Retirement Account (GRA).

Schedule

Normal working hours for this full-time regular position are scheduled between 7:00 a.m. and 5:00 p.m., Monday through Friday, requiring a minimum of a 40-hour workweek, based on a 260-day work calendar. Scheduled special activities, emergencies, or temporary schedule changes may require hours outside of the regular workday or workweek. Evening and weekend availability is an occasional part of the job.
