Job Title: Social Worker - LCSW Job Location: Washington, DC Job Duration: 2-3 Months (possibility of extension) Shift: Monday - Friday, 8:30am - 5:00pm Payrate: $44/ hr. on W2 Job Summary: Fulfills the needs of patients, families and community by providing Social Work services in accordance with his/her licensure, certification, training, and regulatory requirements. Assumes the responsibility for coordinating and facilitating services for patients during and subsequent to admission to the acute rehabilitation unit. Conducts discharge planning, coordination of care, and other planning and counseling activities to effectively move the patient through the continuum of care in a cost-effective and resource efficient manner. Responds effectively to new and changing work assignments. Experience Required: At least 1 year of hospital case management (discharge planning) experience required. Knowledge of client. Certifications Required: LICSW, active DC SW license. Other Requirements: Must live 50 miles away.
