Job Description
The primary role of the Scholar Success Facilitator is to work with scholars, parents, and related social service agencies to help Leman scholars achieve academic success. Counsel scholars and serve as the scholars' advocates. Additional functions include that of consultant, resource person, and community liaison. The Social Worker is competent in the areas of social work, counseling, and consultation. He/she demonstrates the ability to use counseling theory and conducts himself/herself in accordance with district and school policies. He/she possesses comprehensive knowledge of the community's social service agencies.
Minimum Requirements, Education, and Experience
Completed Application (Required)
Master's degree in social work or equivalent professional social work degree from an accredited graduate school of social work (ie. Family Counseling, Counseling, or Psychology)
(1) years of employment as a school social worker and one (1) year of individual and group social work with children and families
Must be knowledgeable about accessing community-based social services and resources
Working knowledge of the principles, practices and ethical standards of School Social Work; the relevant state and federal laws, rules and regulations; and applicable district policies.
Qualifications Preferred:
2 years prior social work experience working with elementary and/or middle school scholars
Prior experience with community collaborations
Prior supervisory experience
Prior experience with program development
Bilingual English/Spanish
Licenses and Certifications:
Valid Arizona Social Worker License upon hire (Required)
Arizona IVP Fingerprint Clearance Card / Background and Criminal History Clearance (Required)
First Aid and CPR Certification (Required)
Services to Scholars:
Counsels scholars whose behavior or school progress indicates the need for individual or small group support
Provides crisis intervention
Assists scholars with conflict resolution and anger management strategies
Helps scholars develop appropriate social emotional interpersonal skills
Assists scholars in understanding and accepting self and others
Service to Families/Parents:
Works with parents to facilitate support for their scholar's school adjustment
Alleviating family stress to enable the scholar to function more effectively in school & community
Assisting parents to access programs available to scholars with special needs
Assisting parents in accessing and utilizing school and community resources
Services to School Personnel:
Providing staff with essential information to better understand factors (cultural, societal, economic, familial, health, etc.) affecting a scholar's performance and behavior
Assessing scholars with mental health concerns
Developing staff in-service training programs
Assisting teachers with behavior management
Providing direct support to staff
Collaborates with school based team to develop intervention strategies to increase academic success
School Community Liaison:
Obtaining and coordinating community resources to meet scholars' needs
Helping school districts receive adequate support from social and mental health agencies
Advocating for new and improved community/school service to meet the needs of students and families
Helping the system respond effectively to each scholar's needs
Related Services:
Participating in exceptional education assessment meetings as well as individual Educational Planning Meetings
Working with those problems in a scholar's living situation that affect the scholar's adjustment in school (home, school, and community)
Preparing a social or developmental history on a scholar with a disability
Counseling (group, individual and/or family)
Mobilizing family, school, and community resources to enable the scholar to learn as effectively as possible in his or her educational program
Assisting in developing positive behavioral intervention strategies
Services to Schools and District:
Assist in developing and implementing educational programs for scholar for exceptional children
Developing alternative programs for at-risk scholars
Identifying and reporting child abuse and neglect
Providing case management for scholars and families requiring multiple resources
Mental and Physical Requirements
