Job Title: LCSW - Social Worker - Clinical
Location: Washington, DC 20037

Duration: 13 Weeks
Shift: 5x8-Hour Day (8:30 AM 5:00 PM)
Hours: 40 hours per week

Requirements:

• Must live 50 miles away
• Minimum 1 year of hospital case management (discharge planning) experience required
• Knowledge of Cerner required
• Active LICSW and DC Social Worker license required

Job Responsibilities:

• Provide social work services to patients, families, and the community
• Coordinate and facilitate patient care during and after admission
• Handle discharge planning and care coordination efficiently
• Support patients through the continuum of care
• Adapt to changing work assignments as needed
