REHAB SOCIAL WORKER / CASE MANAGER
91-Bed Skilled Nursing & Rehabilitation Facility

Our Rehab Unit typically carries a 20–25 guest caseload, giving you the opportunity to truly know the people you’re working with and make a meaningful impact during their stay.
YOUR MISSION:
Help our rehab guests successfully navigate their journey from ADMISSION ️ REHAB ️ HOME!
IN THIS ROLE YOU WILL:
Manage your rehab caseload from admission through discharge
Complete assessments and care plans and facilitate 72-hour conferences
Lead and coordinate discharge planning conferences
Work closely with residents and families
Coordinate referrals to outside agencies and community resources
Arrange ancillary services and follow-up care
Identify barriers to discharge and help find solutions
Collaborate with nursing, therapy, providers, and the entire interdisciplinary team
Make sure each guest has the resources and support needed for a safe transition home
WHO WOULD BE A GREAT FIT?
We prefer candidates with:
️ Licensed Social Worker experience
️ OR LPN with LTC/SNF experience
️ OR Bachelor’s Degree in Social Work, Human Services, Psychology, or another related field
️ LTC/SNF experience strongly preferred

Overtime after 8 hours!The Social Services Director oversees and directs the daily operations of the social services department. This position is responsible for leading efforts to provide social services to residents and their families to enable them to deal with the impact of illness and extend placement. The Social Worker participates in resident care management and plays a key role in advocating and promoting resident rights.

How you will help people every day:- Oversee and direct resident care management and ensure all established social services provisions are carried out.
- Identify medically-related social services needs of residents and ensure that these needs are met.
- Assist in meeting the psychological needs of each resident, including developing and managing services to improve social functioning, reduce psychosocial problems, enhance positive self-image, and preserve dignity.
- Coordinate psychiatric services and monitor psychoactive medication usage.
- You will promote hospitality and excellent customer service in our every daily interaction with our guests, their families, and visitors. Through kind words and encouragement, you will promote a positive, healthy environment in which to heal and thrive.
- You will develop & deliver clinical orientation and training for new nursing team members including classroom training on specific nursing policies, procedures, and techniques; observing, assisting and assessing new team members performance; and ensure new members are equipped with the skills necessary to be successful in their new roles.
What you will need to be part of our EPIC team:- Bachelor’s Degree in Social Work, or in related human services field, including but not limited to Sociology, Special Education, Rehabilitation Counseling, and Psychology;
- Preferred candidate has a Limited License in Social Work; LSW preferred
- 3+ years of related experience in a health care facility; long term care experience
- highly desired; working knowledge of gerontology.
