QUALIFICATIONS 1. A Registered Nurse with a baccalaureate degree in a health care area (or equivalent combination of experience and education), BSN preferred. 2. Maintains license as a Registered Nurse from the State of Michigan. 3. Two (2) to three (3) years nursing experience is required in Hospice, medical, surgical, or critical care experience along with one (1) to two (2) years experience in a supervisory role. 4. Possesses and maintains current BLS certification. 5. Experience with palliative and quality end-of-life care. 6. Presents a negative TB skin test and/or chest X-ray and other tests as required by Centrica Care Navigators. 7. Maintains a valid Michigan drivers license and has the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan. 8. Remains up to date on all routine immunizations required by LARA and agency SUMMARY OF JOB RESPONSIBILITIES The Charge Registered Nurse (Charge RN) provides clinical leadership and coordinates patient care activities during an assigned shift. Is a resource for nursing staff, ensures safe, high-quality patient care, manages patient flow, and promotes effective communication among the interdisciplinary team. Assists leadership with performance and quality initiatives. Demonstrates excellent observation, nursing judgment, and communication skills. RESPONSIBILITIES AND DUTIES Primary Duties Coordinate daily unit operations and patient care activities during assigned shift. Provide strong, visible leadership and support to Residential staff and assist with complex patient care situations. Monitor patient flow, including admissions, transfers, and discharges. Collaborate with physicians, case managers, and other health professionals to ensure continuity of care. Rounds daily with patients and families to provide support, maintain communication on an ongoing basis, and to facilitate coordination of care. Reviews patients medical diagnosis, prognosis, medications, procedures, and clinical course to ensure quality care is provided at appropriate level of care. Assists with overall coordination of patient services provided by the residential staff. Respond to emergencies and coordinate rapid interventions as needed. Assist Residential staff as needed in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing a Plan of Care. Assist Residential staff as needed in completion of new admissions and transfers into the residence. Assist with staff scheduling, low needing or extending. Assist in leading residential staff huddles and initiatives related to operational, performance and quality improvement. Participates in Interdisciplinary Group meetings. Ensure that all IDG notes are completed prior to the meeting Assist with overseeing the maintenance of appropriate inventories of patient-related medications, medical supplies, and equipment necessary for the provision of patient care and services within the hospice residence. Follows all Life Safety regulations and ensures fire drills are completed as directed by maintenance supervisor for the hospice residence. Understands and complies with agency policies and procedures. Understands and complies with the Medicare Hospice Conditions of Participation. Provides guidance and support to staff in developing the skills necessary to effectively plan, prioritize, and manage work assignments to ensure efficient operations and high-quality patient care. Assist the Manager of Centrica Rose Arbor Place by establishing and communicating job responsibilities, performance standards, priorities and expectations to staff. Assist the Manager of Centrica Rose Arbor Place to routinely observe and document both good and poor job performance. Promotes and ensures adherence to Centrica Care Navigators Workplace Safety Program. Ensure that all necessary information is shared with appropriate staff. PHYSICAL REQUIREMENTS A typical ten (10) to twelve (12) hour workday includes intermittent sitting, standing, walking, bending, reaching, twisting, and occasionally carrying 0 - 50 pounds (1 to 33% of time), pushing, pulling, and repetitive hand motions associated with writing and computer keyboard typing. CONTINUING EDUCATION REQUIREMENTS Centrica Care Navigator staff are expected to participate in appropriate continuing education as may be requested and/or required by their immediate supervisor. In addition, staff are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities. All staff must attend mandatory educational programs.
