Position Summary
We are seeking an experienced
RN Manager – Care Management
to lead the Care Management department at an acute care hospital in Petaluma, California.
In this leadership role, you will oversee the daily operations of the Care Management team, ensuring effective care coordination, discharge planning, utilization management, and seamless transitions of care. You will collaborate with physicians, nurses, social workers, and interdisciplinary teams to improve patient outcomes while maintaining quality, compliance, and operational excellence.
Lead the daily operations of the Care Management department.
Supervise, mentor, and develop Case Managers and support staff.
Oversee care coordination, discharge planning, and transition-of-care initiatives.
Monitor department performance, productivity, quality metrics, and regulatory compliance.
Drive quality improvement and patient experience initiatives.
Manage staffing, recruitment, onboarding, performance evaluations, and employee development.
Collaborate with interdisciplinary teams to optimize patient outcomes and efficient resource utilization.
Ensure compliance with organizational policies, accreditation standards, and healthcare regulations.
Bachelor's Degree in Nursing (BSN).
Active California Registered Nurse (RN) license.
Minimum 5 years of healthcare experience.
Minimum 3 years of Case Management experience in an acute care or inpatient hospital setting.
Minimum 1 year of healthcare leadership or management experience.
Master's degree in Nursing, Healthcare Administration, or a related field.
Certified Case Manager (CCM) or Accredited Case Manager (ACM).
