QIDP Case ManagerFull-time hourly 23.00-26.50UCP Seguin of Greater Chicago is seeking a dedicated and compassionate QIDP/Case Manager to join our team. In this role, you will provide essential advocacy and service coordination for individuals with developmental disabilities, ensuring each person's physical, social, emotional, and vocational needs are understood and met. As a leader of the Am- interdisciplinary team, you will guide the development and implementation of Individual Service Plans (ISPs) and collaborate closely with families, service providers, clinical staff, and community partners.The ideal candidate is highly organized, an effective communicator, and committed to empowering individuals with disabilities to achieve independence and community integration. Responsibilities include coordinating services, conducting home and program site visits, monitoring progress, ensuring regulatory compliance, providing crisis support, and maintaining accurate documentation.
