University of Utah Health is seeking a Licensed Clinical Social Worker (LCSW) or Clinical Mental Health Counselor (CMHC) to join the Intensive Outpatient Clinic (IOC), a team-based clinic serving individuals with complex medical and psychosocial needs.

This is not a traditional outpatient behavioral health role focused on billing volume.

The IOC operates within a value-based Medicaid model in partnership with University of Utah Health Plans. Our interdisciplinary team brings together medical providers, behavioral health clinicians, care coordinators, and other clinic staff to deliver integrated, whole-person care designed to improve health outcomes and reduce avoidable emergency department visits, hospitalizations, and total cost of care.

In 2023, the IOC was recognized by Governor Spencer Coxs One Utah Health Collaborative for innovation in comprehensive care delivery. The clinic was also featured in the Sundance Film Festival short film Meet Me Where I Am, which highlights the impact of person-centered, integrated care.

Our Approach

The IOC provides comprehensive, integrated care for patients with complex medical and psychosocial needs. The clinic is part of the University of Utah Population Health Center and works closely with primary care, behavioral health, care management, dental services, dermatology, pediatrics, and other programs to support coordinated care.

As the clinic continues to grow, the IOC is refining its value-based care model in collaboration with University of Utah Health Plans. Clinical care is aligned with measurable outcomes, including reductions in emergency department use, inpatient admissions, and total cost of care.

The Setting

The IOC is located at the University of Utah Population Health Center near Rose Park in Salt Lake City. This is an on-site clinical role supporting both the IOC patient population and the broader clinic team.

This position reports to the IOC Medical Director.

This position is considered Category I: Mandatory and is required on campus or in health care facilities to support on-site operations. Employees hired into this category will be required to be physically located at 168 North 1950 West, Salt Lake City, UT 84116 to support clinic operations.

Responsibilities

What Youll Do

The Social Worker / Behavioral Health Clinician provides direct therapy and works collaboratively with the medical and behavioral health teams to support patients with complex medical, behavioral health, and social needs.

Key responsibilities include:

  • Provide individual and group counseling for IOC patients.
  • Support patients with trauma, substance use disorders, serious psychosocial stressors, and barriers to care.
  • Collaborate closely with medical providers, behavioral health clinicians, care coordinators, and clinic staff.
  • Participate in interdisciplinary case review, care planning, and team-based clinical decision-making.
  • Help improve outcomes for patients who have historically struggled to access effective, coordinated care.
  • Contribute to care model development, quality improvement, and population health initiatives.

This is hands-on clinical work in a setting where teamwork, continuity, and relationship-based care matter.

Minimum Qualifications

Required Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.One of the following

  • Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah.
  • Current license to practice as a Certified Social Worker in the State of Utah.

This position is patient-sensitive and must fulfill all associated requirements.
