Project Coordinator, Transition Services, Family and Community Medicine (Northern AZ)

• Manage day-to-day operations, supervise staff, coordinate and oversee project activities, complete project administrative coordination, and make operational decisions.
• Develop and maintain collaborative partnerships with community rehabilitation providers, school personnel, community partners, rehabilitation counselors, family members, students, employers, and state agency representatives.
• Engage schools and participants statewide, including rural, foster-serving, and Tribal schools.
• The duties require travel to different schools and community events through the course of the week. May require attending events in the evening and/or weekends. May require overnight travel across the state.
• Conduct comprehensive needs assessments for schools and communities, deliver targeted training and technical assistance to build capacity, and collaborate effectively to design and implement work-based learning and summer work opportunities.
• Identify and develop best practices for delivering all five areas of Pre-Employment Transition Services (PreETS), and establish effective strategies to replicate and sustain these practices across various communities, including metropolitan, rural, and tribal areas.
• Develop and provide training and technical assistance on best practices for Vocational Specialists; facilitate brainstorming and creative problem-solving; promote skill development through one-on-one support, mentoring, and informational opportunities; and design products and resources for staff and partners.
• Design and implement methods to gather participant input and feedback. Oversee, monitor, and synthesize data collection for project reporting.
• Maintain and refine implementation strategies, incorporate innovative approaches, and resolve challenges to facilitate the continuous improvement of the PreETS service delivery model and the implementation of best practices around transition with partner schools.
• Collaborate closely with the Program Manager, Transition Specialist, and Project Coordinators, ensuring timely updates and proactively addressing concerns.
• Disseminate information through a variety of written and digital platforms, including social media, presentations, newsletter articles, and webinars, to maximize outreach and engagement.
• Actively participate in project planning meetings and attend relevant state and national events to stay informed about current trends and best practices.
• Perform additional duties as assigned to support the overall success of the Sonoran Center.

Knowledge, Skills & Abilities:

• Strong understanding of Pre-Employment Transition Services (PreETS) and best practices, with the ability to implement these services effectively.
• Ability to travel to school and community partner locations during the regular work week, May include occasional evening (after-hours), overnight, and/or weekend travel.
