Family Health Centers at NYU Langone, a designated Level 3 Medical Home, is the largest federally qualified health center network in New York State, and the second largest in the nation. Position Summary:
We have an exciting opportunity to join our team as a Program Supervisor.

In this role, the successful candidate Develop and implement school based initiative to address social determinants of health and key health related performance indicators in collaboration with clinical, administrative and school leadership. Initiative will integrate community health workers into elementary schools and health educators into high schools with school based health clinics to address the needs of students and their families.

Job Responsibilities:

  • Develop workflows for community health workers and health educators embedded in high school and elementary school based health centers
  • Build relationships with key staff in school based health centers, schools, and partnering community based organizations to ensure that community health workers are successfully integrated into each school.
  • Assess resources and needs in individual schools to inform planned services and protocols.
  • Provide supportive, reflective supervision to community health workers.
  • Ensure that families and students that are most in need of services are referred to community health workers.

  • Work with FHC leadership and the data team to ensure that needed data fields are available..
  • Ensure appropriate documentation of outreach, screening, and case management services to families
  • Ensure community health workers and health educators are screening all families for social drivers of health so that they can navigated to any services they are eligible for.
  • Support CHW staff in screening for families for benefit eligibility and assisting with benefit applications.
  • Use data for continuous program improvement and to track progress towards key performance indicators
  • Assist with planning and facilitation of FSS staff meetings.
  • Provide and/or coordinate staff development for community health workers

  • Collaborate with community partners and health center leadership to develop medically accurate, and culturally competent outreach and education materials
  • Create and sustain relationships with service provides to support successful referrals by staff
  • Ensure that community health workers are screening eligible families for services available through the 1115 waiver and conducting eligibility assessments and navigation

Minimum Qualifications:
To qualify you must have a BA is required. Masters in social work or public health preferred, but not needed. At least two years of experience in providing direct services related to health care or case management and at least one year of experience in providing program management and/or staff supervision.

Preferred Licenses: Social Work Case Mgr -Adv Cert

Preferred Qualifications:
Good interpersonal skills and strong orientation to community practice and strength based intervention models. Bilingual Mandarin or Spanish preferred.

Qualified candidates must be able to effectively communicate with all levels of the organization.

NYU Langone Health provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $66,300.00 - $70,000.00 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses/incentive, differential pay or other forms of compensation or benefits.
