Primary Care Provider (NP/PA) - Community-Based Senior Care
Our organization operates a comprehensive, community-based senior care program designed to empower elderly participants to remain in their homes and communities. We utilize an innovative, whole-person medical approach driven by a collaborative interdisciplinary team. We are seeking a dedicated Primary Care Provider to join our mission-driven culture and lead participant care coordination.
Under the guidance of the Medical Director, you will provide clinical assessments and primary care while working alongside a diverse team of healthcare professionals. This role is essential for maintaining high-quality medical coverage and continuing education for our program participants.
Core Responsibilities:
Conduct in-person comprehensive medical assessments
Partner with the interdisciplinary team to design and implement individualized care plans.
Manage discharge planning for participants transitioning from acute or long-term care settings.
Oversee participant care within nursing facilities, providing regular clinical visits.
Facilitate effective care coordination with community-based physicians and specialists.
Analyze utilization data and hospitalization outcomes to improve clinical policy and best practices.
Participate in a rotating schedule for night and weekend on-call coverage.
Requirements
Candidate Qualifications:
Degree from an accredited School of Nursing (NP) or Physician Assistant (PA) program.
Current California State License as a Nurse Practitioner (with prescriptive authority) or Physician Assistant.
Minimum of two years of clinical experience specializing in the geriatric population.
Strong proficiency in chronic care management and geriatric best practices.
