Primary Care Nurse Practitioner / Physician Assistant

Novant Health is seeking a Primary Care Advanced Practice Provider (APP) to join our growing team in South Carolina. This is an excellent opportunity for a Nurse Practitioner or Physician Assistant who is passionate about providing comprehensive, patient-centered primary care and building long-term relationships with patients and families.

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Current NP or PA certification and South Carolina licensure/eligibility
• Family Medicine, Internal Medicine, Adult-Gerontology, or related primary care experience preferred
• New graduate candidates with strong clinical training may be considered
• BLS certification required
• Excellent communication and interpersonal skills
• Commitment to high-quality, patient-centered care

• Provide comprehensive primary care to patients across the lifespan
• Perform preventive, routine, and problem-focused evaluations
• Diagnose and manage acute and chronic medical conditions
• Develop individualized treatment plans and provide ongoing care management
• Order and interpret appropriate diagnostic testing
• Provide preventive health education, counseling, and screening
• Coordinate care with physicians, specialists, and other members of the care team
• Promote continuity of care and strong patient-provider relationships
• Participate in quality improvement and population health initiatives
• Support the continued growth and development of the primary care practice
