Compensation:

• $60 – $65 per hour

Position Summary & Responsibilities (SNF)

• Evaluate and treat SNF residents

• Provide and document skilled therapy services

• Ensure Medicare/PDPM compliance

• Collaborate with interdisciplinary care team

• Maintain productivity and quality standards

Requirements

• Active Physical Therapist (PT) license

• SNF or post-acute rehab experience required

• Knowledge of Medicare/PDPM guidelines and documentation

• Strong communication and teamwork skills
