JOB OVERVIEW
Seeking a dedicated Physical Therapist (PT) to provide restorative and rehabilitative physical therapy services as part of an interdisciplinary healthcare team. The Physical Therapist will assess patient needs, develop individualized treatment plans, and deliver therapeutic interventions designed to maximize independence, safety, mobility, and quality of life.

KEY RESPONSIBILITIES

Conduct physical therapy evaluations and assessments to determine patient needs, goals, and treatment plans
Participate in interdisciplinary team meetings and contribute to individualized care planning
Deliver and document physical therapy interventions in accordance with established care plans
Provide active and passive exercise, muscle re-education, gait training, balance training, and functional mobility interventions
Deliver therapeutic treatments and modalities as clinically appropriate
Evaluate patient progress and modify treatment plans as needed
Educate patients, caregivers, and staff regarding therapeutic techniques, mobility strategies, and equipment use
Supervise and provide guidance to Physical Therapist Assistants and support staff as appropriate
Maintain accurate and timely clinical documentation

REQUIREMENTS

Bachelor s Degree in Physical Therapy required; Master s or Doctoral Degree preferred
Active Physical Therapist License or eligibility for licensure in the applicable state
CPR and First Aid Certification preferred upon hire and required within designated timeframe if not already certified
Strong clinical assessment, treatment planning, and documentation skills
Ability to work effectively within an interdisciplinary healthcare team

PREFERRED EXPERIENCE

1-2 years of Physical Therapy experience
Experience in outpatient rehabilitation, acute care, inpatient rehabilitation, or skilled nursing settings
Experience treating adult and geriatric populations
Experience working with medically complex patients
