Pay & Benefits:

• Hourly Pay Rate: $28 – $37/hour (dependent on experience)

What You’ll Do:

• Conduct comprehensive nutritional assessments for residents at risk or as needed
• Develop and oversee individualized nutritional care plans
• Coordinate nutrition services in collaboration with medical staff and other caregivers
• Monitor and evaluate residents' weight, wound care, and overall nutritional status
• Participate in quality improvement and survey readiness activities
• Document assessments and care plans accurately in electronic records
• Provide nutritional education to patients and their families
• Contribute to a multidisciplinary team to promote patient-centered care

What You Need:

• Valid Registered Dietitian (RD) license in Vermont or ability to obtain
• Prior experience with long-term care and inpatient settings preferred
• Strong communication and teamwork skills
• Ability to assess nutritional needs across diverse populations
