The role includes responsibilities for establishing and monitoring continuous quality improvement issues per regulations. Nutritional instruction is provided to patients and documented appropriately in the medical record. The Dietitian coordinates care with nutritional staff members and medical staff physicians. A multi-disciplinary approach is utilized for the care of the patient. Striving for superior performance is essential by providing recognized services that contribute to the patient and family experience. Understanding of patient and family-centered care is crucial. Work attire includes black scrubs or business professional. Primary responsibilities include conducting comprehensive nutrition assessments for patients identified at nutritional risk. The Dietitian incorporates evidence-based practices when implementing nutrition interventions. Evaluation of the need for specialized nutrition support and provision of recommendations is required. Appropriate documentation of nutrition assessments in the electronic medical record is necessary. Reassessing the nutrition care process and implementing changes as indicated is an important responsibility. The Dietitian recognizes the varied needs of age-specific populations, along with cultural, religious, and ethnic concerns. Developing and directing the overall nutrition and hydration plan of care for each resident is expected. Evaluation of residents for significant weight changes, new admissions, new or continuing wounds, and participation in care conferences is required. Quality improvement project participation is needed as necessary. Familiarity with clinical, outpatient, and classroom instruction experience is highly preferred. Community service nutritional experience is also highly preferred. On-call responsibilities are required every other weekend. Working on-site is expected during the on-call weekend.
