About us Sonoran Family Concierge is a veteran- and woman-owned, non-medical in-home care agency serving Scottsdale, Paradise Valley, Fountain Hills, Tempe, Mesa, Gilbert, Chandler, and Greater Phoenix. We were founded on a simple promise: the same caregiver every visit, and the owner personally involved in every case. We’re a proud member of the Arizona In-Home Care Association.
The role You’ll be the face of Sonoran Family Concierge in the professional community. You’ll build and maintain relationships with the people families turn to when a parent needs help at home: hospital and SNF case managers, senior living directors, physician practices, hospice teams, elder law attorneys, and fiduciaries. This is relationship work, not cold-call volume. You’ll work directly with the owner, and your success is measured by trusted referral partnerships that turn into families we serve well.
What you’ll do
- Visit and build ongoing relationships with referral partners across the Valley (hospitals, rehab/SNF, assisted and independent living, physician offices, hospice, elder law)
- Manage a portfolio of partner accounts with regular touchpoints, in-services, and follow-up
- Represent SFC at networking events, community breakfasts, and senior expos
- Respond quickly to new referrals and hand them off smoothly to the owner for assessment
- Track all activity and follow-ups in our CRM and share weekly updates
- Bring back what you hear in the field so we keep improving
What we’re looking for
- 2+ years in senior care, home health, hospice, senior living, or healthcare community relations (existing Valley relationships are a big plus)
- A warm, polished, professional presence. You’ll be speaking with adult children making hard decisions about their parents.
- Self-directed and organized, comfortable working independently in the field
- Strong follow-through and comfort with a CRM or tracking tools
- Valid Arizona driver’s license, reliable vehicle, and current auto insurance
- Ability to pass a background check (Arizona Fingerprint Clearance Card preferred)
Compensation & schedule
- $17-18]/hour base plus a bonus for each new client who starts service and stays [30/60] days
- Mileage reimbursement
- Approximately 15-20 hours/week, weekday daytime, with a flexible schedule
- Target start: December 2026 - January 2027
