Pay From: $55.00 per hour


MUST:
Current, unrestricted Nurse Practitioner license in the state of Virginia
Current national board certification as a Nurse Practitioner through an appropriate recognized certifying organization.
Current Basic Life Support (BLS) certification.
Eligible for or currently holding applicable prescriptive authority and controlled-substance registration, as required.
Clinical experience in internal medicine, primary care, family medicine, or a related adult healthcare setting preferred.
Strong clinical assessment, diagnostic, communication, and organizational skills.
Experience with electronic medical records.
Experience with chronic disease management and preventive healthcare.
Experience working in a multidisciplinary healthcare environment.

DUTIES:
The Nurse Practitioner (NP) provides comprehensive, high-quality, patient-centered care to adult patients in an Internal Medicine Medical Center. The NP is responsible for evaluating, diagnosing, treating, and managing patients across a broad range of acute and chronic medical conditions while emphasizing preventive care, health education, and long-term wellness.
The Nurse Practitioner works collaboratively with physicians, nurses, medical assistants, and other members of the healthcare team to ensure coordinated, timely, and compassionate patient care.
Essential Responsibilities
  • Conduct comprehensive medical histories and physical examinations.
  • Assess, diagnose, and develop individualized treatment plans for adult patients.
  • Manage common acute illnesses and chronic medical conditions, including hypertension, diabetes, hyperlipidemia, asthma, COPD, and other conditions commonly seen in internal medicine.
  • Order, interpret, and follow up on laboratory tests, diagnostic studies, and other appropriate evaluations.
  • Prescribe medications and treatments in accordance with applicable laws, regulations, and organizational policies.
  • Monitor patient response to treatment and modify treatment plans as clinically appropriate.
  • Provide preventive healthcare services, including health screenings, immunizations, wellness counseling, and risk-factor assessment.
  • Provide patient education regarding diagnoses, medications, lifestyle modifications, disease prevention, and self-management.
  • Perform appropriate in-office procedures within the NP's scope of practice, training, and clinical privileges.
  • Coordinate referrals to specialists and communicate with other healthcare providers as needed.
  • Review and follow up on diagnostic results and ensure appropriate patient communication.
  • Maintain accurate, timely, and thorough documentation in the electronic medical record (EMR).
  • Participate in care coordination and population health initiatives as appropriate.
  • Collaborate with supervising or collaborating physicians and other healthcare professionals regarding complex patient needs.
  • Recognize and appropriately manage patients requiring urgent or higher levels of care.
  • Maintain compliance with applicable federal and state regulations, medical center policies, and professional standards.
  • Participate in quality improvement, patient safety, and clinical performance initiatives.
  • Maintain professional knowledge through continuing education and ongoing clinical development.
