In this role, you will provide care coordination and health education services for individuals with complex medical conditions or those at risk for developing chronic health concerns. KeyResponsibilities:

·Coordinatecomprehensivehealthcareservicesandcollaboratewithinternalandexternaltreatmentteammemberstosupportpositiveclientoutcomes

·DevelopindividualizedhealthcaregoalsandcontributetotreatmentplanningforHealthcareHomeenrollees

·Facilitatehealtheducationgroupsandprovidetrainingtoindividualsregardingmedicaldiagnoses,treatmentplans,medications,andchronichealthconditions

·Promotewellnessinitiativesandpreventativecarestrategiesthatsupportlong-termhealthandwellbeing

·Consultwithtreatmentteammembersregardingidentifiedhealthconcernsandrecommendappropriateinterventions

·Managehospitaladmissions,discharges,andtransitionsofcaretoensurecontinuityofservices

·Utilizehealthcaretechnologyplatformsandsystemstosupportcaremanagementactivitiesandprograminitiatives

·Monitor,analyze,andreportperformancemeasuresandhealthoutcomesusingestablishedinformationsystems

·Respondappropriatelyduringemotionalormedicalcrisesandcoordinateemergencyserviceswhennecessary

·Participateinprogrammeetings,qualityassuranceactivities,programevaluations,andotherorganizationalinitiatives

·Maintaincompliancewithorganizationalpolicies,professionalnursingstandards,clinicaldocumentationrequirements,andethicalguidelines

Education,Experience,and/orCredentialQualifications:

·ActiveRegisteredNurse(RN)license

·ExperienceworkingwiththeIntellectualandDevelopmentalDisabilities(IDD)populationpreferred

·Knowledgeofchronicdiseaseprocessesaffectingchildrenandadults

·Strongunderstandingofnursingprocessesandclinicaldecision-making
