Home Health Medical Social Worker (MSW) – Per Visit / PRNHope at Home Health Care – Southfield, MI (field-based)
About usHope at Home Health Care is a Medicare-certified home health agency based in Southfield. We serve patients throughout Oakland, Macomb, Wayne and Livingston Counties. Our interdisciplinary team helps patients recover safely at home after a hospitalization, an illness or an injury.
The roleWe’re looking for a Medical Social Worker to make home visits to our patients on a flexible, per-visit basis. You’ll work alongside our nurses and therapists to address the social, emotional and financial issues that stand in the way of a patient’s recovery and independence.
Responsibilities
- Complete psychosocial assessments in patients’ homes and help develop the plan of care
- Connect patients and families with community resources: Medicaid, Medicare, MI Choice waiver, Area Agencies on Aging, Meals on Wheels, transportation, housing and caregiver support
- Provide short-term counseling for adjustment to illness, caregiver stress, grief and end-of-life decisions
- Assist with advance directives, long-term care planning and placement
- Identify and report concerns about safety, abuse, neglect or self-neglect
- Document visits accurately and on time in Kinnser/WellSky
- Communicate with the clinical manager and the interdisciplinary team, and take part in case conferences
Requirements
- Master’s degree in Social Work (MSW) from a CSWE-accredited program
- Active Michigan LMSW license
- At least 1 year of social work experience in a health care setting
- Valid driver’s license, reliable transportation and auto insurance
- Current CPR/BLS certification
Preferred
- Experience in home health or hospice
- Knowledge of Medicare home health and community resources in Southeast Michigan
- Experience with an EMR (Kinnser/WellSky a plus)
Pay & schedule
- $70-$80 per visit
- Flexible PRN scheduling: you choose your availability and coverage area
