Sante Health System is seeking an exceptional Medical Assistant to join our team in Fresno, CA. This role is responsible for collaboration with staff, physicians and management to perform and achieve objectives of the Chronic Care Management program for all eligible patients according to insurance guidelines. This Medical Assistant also promotes patient engagement and satisfaction, ensure outcome quality metrics are met and reimbursements are captured. Knowledge of organizational policies, regulatory requirements and procedures to administer patient care. Knowledge of common safety hazards and precautions to establish a safe work environment and evaluate same for home care. Skill in health interviewing techniques, teaching and communication both verbally and in writing. Skill in identifying problems and recommending solutions utilizing established protocols and a common-sense approach. Skill in preparing and maintaining records, writing reports and responding to correspondence. Skill in developing and maintaining site quality assurance. Skill in establishing and maintaining effective working relationships with patients, medical staff and the care team members. Ability to react calmly and effectively in emergency situations. ESSENTIAL DUTIES AND RESPONSIBILITIESDevelops an efficient process for Chronic Care Management (CCM), to include: CCM identification, communication, scheduling and follows up through collaboration with physicians and office staff. Supports identification of eligible patients for CCM and completes the CCM monthly calls according to best practice guidelines. Ensures that CCM calls are coded accurately for maximum financial capture. Provides age-based, culturally and language appropriate patient education. Assists patients through the healthcare system and community by acting as a patient advocate and navigator. Encourages and facilitates patient self-management of disease, behavior modification interventions, and home monitoring within the scope of Chronic Care Management. Maintains the sustainability and process improvement for Chronic Care Management and makes referrals as appropriate. Demonstrates System Values in performance and behavior. Complies with System policies and procedures. Complete comprehensive, age appropriate review based on patient strengths, goals and needs, acute and chronic health conditions, environment and psychosocial determinants of health. Conduct ongoing response to care plan or services and/or changes in patient's biophysical, psychosocial, family or environment status. Notify RN/LVN on new information and assure patient, family and care team members who participate are informed of new information. Provide general information on department policies and procedures. Maintain patient files, records, and other information. Attend required meetings and participates in committees as requested. Participate in professional development activities and maintain professional affiliations. Follow all MA competencies. Maintain patient confidentiality in compliance with HIPAA regulations.
