    Description

    Requirements: License and certifications:
    RAI manual expertise required with certification
    • Registered Nurse with current Nebraska nursing license or from a compact state recognized by the Nebraska
    • Board of Nursing upon hire and maintained in good standing throughout employment required.
    • Three (3) years of clinical experience or clinical operations expertise in a LTC/Skilled Rehabilitation health care environment required. *
    • Two (2) years of experience completing MDS assessments and care planning required including an extensive
    • Knowledge of the RAI manual, RUG scores, Prospective Payment System (PPS) assessment, PDPM, care planning, CAA's, OBRA's and regulatory requirements related to LTC required.
    • Completion of Resident Assessment Coordinator-Certified (RAC-CT) within six (6) months of hire required.
    • Basic life support/cardiopulmonary resuscitation (CPR) certification within three (3) months of hire and maintained throughout employment required
    Responsibilities:
    • Complete Nursing areas of responsibility on the MDS assessments, remaining alert and current on completion/submission requirements/timelines for MDS assessments (e.g. VA contracts, managed care plans) and submitting completed MDS assessments to the State.
    • Develop, document, and implement individualized interdisciplinary care plans ensuring the patients'/residents' care/medical needs.
    • Establish and maintain effective work relationships with clients, supervisors, County employees, elected officials, attorneys, law enforcement, judges, other agencies, and the public.
    • Comply with Civil Service policies and regulations, collective bargaining agreements, County policies, department policies and laws to create a cooperative, safe, respectful and quality work environment.
    • Gather information, review chart documentation, and interview patients/residents, ensuring completion and documentation of follow-up and assessment data is recorded.
    • Assist in developing and revising MDS and care planning policies and procedures, ensuring regulatory compliance and providing ongoing support and training. Work with departments ensuring proper coding of the MDS.
    • Ensure disciplines are informed of changes in the Resident Assessment Instrument (RAI) manual. Provide information exchange relating to assessments amongst disciplines and the MDS Coordinator.
    • Adhere to facility interpretation of the MDS regulations and standards ensuring compliance with local, state, federal, and other regulatory agencies.
    • Participate in developing, implementing and evaluating the plan of care, attending Care Plan meetings and ensuring care plans are updated and reflect individual care needs.
    • Assist in informing the interdisciplinary team of assessment due dates ensuring achievement of required timelines.
    • Maintain knowledge and job skills (e.g. journals, workshops, seminars, webinars, continuing education, meetings) informing management of changes.
    • Establish and maintain contacts with MDS professionals from other health care organizations (e.g. American Association of Nursing Assessment Coordinators (AANAC)), exchanging information and remaining current with regulatory requirements.
    • Assist in providing annual in-service education and orientation programs. Attend and/or lead facility meetings, consult and assist with MDS completion, scheduling, various payment type classifications, Omnibus Budget Reconciliation Act (OBRA), and appropriate therapy dates.
    • Review and verify payment sources ensuring information is accurately entered. Serve as a resource person, promote a positive work environment, and assist in resolving conflict.
    • Maintain confidentiality of department, patient/resident, employee information, communication, and documentation and compliance with Health Insurance Portability and Accountability Act (HIPPA) regulations.
    • Inform Nursing and Therapy of changes in patients'/residents' condition, continue communication, and immediately report any suspicion/allegation of abuse/neglect to supervisor.
    • Promote and maintain a clean and safe work environment ensuring malfunctioning equipment is removed and tagged for repairs.
    • Respond to and assist with medical and psychiatric emergencies and codes. Observe, adhere to, and promote emergency/safety protocols and measures related to hazards (e.g. bio, chemical, fire, weather, internal disasters, wandering patients/residents, community disasters).
    • Provide technical assistance to staff (e.g. MDS assessments, new employee orientation, clinical nursing preadmission assessment).
    • Participate in Quality Assurance and Performance Improvement (QAPI) activities. Perform appropriate direct hands-on nursing care and provide patient/resident education.
    • Plan, coordinate, and supervise nursing care ensuring compliance with standards of care, policy/procedures.
    • Assess patients/residents, initiate physician orders, administer medication, complete procedures, and provide treatments.
    • Document assessments, care, education, and observations in medical record
    • Report on work with regular, consistent attendance.
    • Perform other duties as assigned and directed
