Summary:
Duration: 5 months contract with possible conversion but no guarantee
Work Mode: Remote
Location: Durham & Wake counties, NC - Region 4, EST zone
Shift: 8am-5pm EST, Monday – Friday
Responsibilities:
Develop, assess, and coordinate holistic care management activities to support quality, cost-effective healthcare outcomes.
Create or support personalized service plans for long-term care members.
Educate members, families, and caregivers about available services and benefits.
Evaluate the needs of the member and recommend or facilitate the best outcome plan.
Assist in developing ongoing long-term care plans/service plans and identify necessary providers and resources.
Coordinate between members, families, caregivers, and the care provider team to ensure service accessibility.
Provide resource support for various needs based on service assessment and plans.
Monitor care plans/service plans and provide recommendations based on member needs.
Interact with healthcare providers and partners to ensure member needs are met.
Collect, document, and maintain member information and care management activities for compliance.
Perform home or site visits to assess member needs and collaborate with healthcare providers.
Provide education on procedures, healthcare provider instructions, service options, referrals, and benefits.
Offer feedback to leadership to improve quality of care and service delivery.
Requirements:
Requires a Bachelor's degree and minimum 2 years of related experience.
Licensure: RN or LCSW required.
Minimum 2 years of in-home experience.
Experience in LTSS and/or HCBS coordination, care delivery monitoring, and care management.
Experience in social work, geriatrics, gerontology, pediatrics, or human services.
Reliable access to own transportation.
Preferred Skills:
Prior managed care experience.
Ability to work autonomously.
Strong clinical documentation and computer skills.
