Job Description:
AHMG Palliative Medicine and Supportive Care at Orlando
200 N Lakemont Ave, Winter Park, FL 32792
Monday - Friday 8am - 5pm
- Performs psychosocial assessments and provides psychotherapy to patients and families coping with medical issues.
- Maintains comprehensive community resources and networks to assist patients and families as they transition back to the community.
- Collaborates with the multi-disciplinary team to establish and document psychosocial determinants for safe discharges.
- Serves as a consultant and educator to the multi-disciplinary team regarding the psychosocial components of illness, disability, and hospitalization.
- Facilitates staff emotional debriefing and ongoing support groups.
- Applies knowledge of local, state, and federal programs to assist with appropriate referrals and coordination of post-acute care.
- Participates in the development of programs to support patients and families dealing with complicated medical issues.
- Attends physician rounds and forums to facilitate communication and clinical social work input to the interdisciplinary team.
- Demonstrates working knowledge of the involuntary commitment process. Provides counseling services, crisis intervention, stress management, and bereavement support.

Work Experience:

- Of direct clinical with case management experience preferable in a health or mental health setting with an emphasis on mental health, substance abuse and/or geriatric population [Required]
