As a Care Coordinator, you'll serve as the connection between our clients, families, caregivers, and nurses, helping ensure the right care is delivered at the right time.

What You'll Do

Coordinate daily schedules for caregivers and nursing staff
Build and maintain strong relationships with clients and families
Communicate schedule changes and care updates promptly
Support and assist field staff with day-to-day needs
Investigate and resolve client and employee concerns
Maintain accurate documentation and reporting
Partner with clinical and operational teams to ensure quality care
Assist with ongoing quality improvement initiatives

Phoenix provides Home Health Care services to those who desire to remain independent at home. Phoenix provides acute Skilled Nursing through Home Health Services; Private Duty Nursing to Children with Special Needs; Privately Paid Services such as nursing, nurse-aide or companion level assistance with activities of daily living from a few hours a day to around-the-clock care; State funded In-home Services (homemaker chores, errands and shopping, personal care, education, medication set-ups and reminders; and Consumer Directed Services where Phoenix serves as a state vendor to provide administrative support services.
