Hospice Liaison, Bringing Dignity and Care to Every Patient Journey. As the Hospice Liaison, you'll serve as a trusted resource in the community, building relationships with healthcare providers and guiding families toward compassionate hospice services. This role is more than marketing, it's about ensuring dignity, comfort, and support during life's most challenging moments.
Responsibilities
Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources
Educate healthcare professionals and community groups about hospice services and benefits
Serve as a knowledgeable, compassionate resource for patients and families during the referral and admission process
Document outreach activities and maintain accurate records of interactions
Ensure all messaging reflects our commitment to dignity, comfort, and quality care
Collaborate with care coordination teams to support smooth transitions for patients
Monitor community needs and identifies opportunities to improve access to hospice care
Requirements
Bachelor's degree in Marketing, Business Administration, or related field preferred
Minimum 3 years of healthcare marketing experience, hospice experience strongly preferred
Strong communication, relationship-building, and public relations skills
Ability to work independently while maintaining a compassionate, patient-first approach
Demonstrated organizational skills and ability to meet outreach goals
Reliable transportation, valid AZ driver's license, and proof of insurance
