    Description: Milford Public Schools is seeking a multilingual Registered Nurse (RN) (Portuguese and/or Spanish preferred) to work full-time in our Family Welcome Center. The RN will reduce barriers to timely health care by connecting all enrolling/registered families/students of Milford Public Schools (i.e., newly immigrated families, McKinney-Vento, etc.) to available community health resources through proactive, personalized outreach and follow-up.

    Qualifications: Valid license to practice as a Registered Nurse in Massachusetts Valid MA DESE licensure as a School Nurse or eligible for this license Minimum of a Bachelor of Science in Nursing Current certification in cardiopulmonary resuscitation/AED and first aid from a recognized provider. At least three to four years of nursing experience, preferably in school nursing or other community or public health setting.     Roles and Responsibilities: Collaborate with the Family Welcome Center (FWC) Director and the MPS Director of Nursing (DON) to identify and prioritize families/students who need connections to local health resources. Work as part of a healthcare team alongside MPS school nurses, MPS clinical social workers, MPS guidance counselors, the MPS district physician, and local pediatric offices to deliver quality patient care. Connect with local healthcare resources and other partners to identify reliable referral options for free or low-cost care. Review newly enrolled student health records to identify areas of unmet health needs and expedite the connection of families without medical insurance to appropriate free or low-cost local health care resources and/or community health centers. Provide vision screening in accordance with MA DPH guidelines for newly registering immigrant students during the registration process to early identify possible vision deficits that may impact learning, and coordinate with MPS DON or school nurses for referrals for failed vision screenings to eye care specialists. Attend monthly community and state-level network meetings, such as, but not limited to, Milford Area Resource Meetings, Health Care For All Working Group, and Immigrant Health Access Coalition Identify any barriers to healthcare access and collaborate with the MPS DON to determine a plan of action. Assist families in obtaining the required preschool entry physical examinations, immunizations, and other medical documentation for school entry; follow up with these families to ensure timely completion of the requirements. Assist families who have had a change in health coverage in finding a new local provider. Connect with local healthcare resources and other partners to identify reliable referral options for free or low-cost care. Assist students'' families with vision referrals to eye care specialists and dental referrals to local dental specialists. Coordinate MPS immunization clinics with the Milford Board of Health and the MPS DON for registering students in need of immunizations. Facilitate vaccine clinic appointments in order of priority to ensure timely school entry. If necessary, assist with vaccine administration in accordance with the BOH standing orders. Vaccine management: Handle and store vaccines in accordance with manufacturer guidelines and protocols. This includes inventory management and reconciliation. Input medically cleared or conditionally cleared students into an Immunization log and send folders to the corresponding school nurses. Connect with MPS school nurses to identify newly registering and/or recently registered students who may still lack physical examinations and/or immunizations after starting school, and provide a timeline and plan to facilitate completion of these requirements. In coordination with the MPS DON, develop written materials to provide registering families with essential, relevant health information and resources. Additional associated responsibilities as per the MPS Superintendent, Director of the FRC, or MPS DON.
