The Director of Assisted Living & Independent Living Operations is responsible for the development, implementation, and ongoing operational management of physician and advanced practice provider services within Assisted Living (AL), Independent Living (IL), and related senior living settings.

This position will lead the development of a scalable AL/IL service line while recognizing that the clinical and operational model may differ significantly based on the type of community, state regulations, available clinical infrastructure, access to medical records/EHR systems, patient needs, and the organization's relationship with the patient's existing Primary Care Provider.

The Director will work closely with Clinical Leadership, Compliance, Legal, Revenue Cycle, Credentialing, Information Technology, Population Health, and Operations to ensure services are clinically appropriate, operationally efficient, compliant, financially sustainable, and capable of expansion across multiple markets.

    Key Responsibilities

    Service Line Development & Implementation

Lead the development and expansion of the organization's Assisted Living and Independent Living service line. Establish scalable operating models, implementation standards, workflows, and infrastructure necessary to successfully launch and manage services across multiple communities and markets.

Evaluate new opportunities and coordinate cross-functional implementation efforts to ensure clinical, operational, financial, regulatory, technology, credentialing, and billing requirements are addressed prior to launch.

    Care Delivery Models

Develop and operationalize different models of care based on the needs and capabilities of each senior living environment.

Models may include traditional longitudinal primary care, scheduled provider rounding, same-day sick or episodic visits, specialty services, and other appropriate care-delivery models.

Ensure that workflows clearly define the organization's clinical role, particularly when providing episodic or same-day services to residents who maintain an established PCP outside of the organization.

Assisted Living vs. Independent Living Operations

Develop appropriate operational approaches for Assisted Living and Independent Living settings, recognizing differences in licensing, clinical staffing, nursing availability, medication management, resident independence, medical record availability, and facility capabilities.

Assess each location to determine the most appropriate model of care and operational infrastructure before services are initiated.

    Clinical Information & EHR Workflows

Establish processes that allow providers to safely and effectively evaluate patients regardless of whether the organization has direct access to an electronic medical record.

For locations without EHR access, work with clinical leadership to develop standardized intake, documentation, consent, medical history, medication reconciliation, information exchange, and follow-up processes that provide clinicians with the information necessary to appropriately evaluate and treat patients.

    Regulatory & Compliance

Maintain a strong working knowledge of the regulatory requirements associated with providing physician and APP services in Assisted Living and Independent Living settings.

Partner with Compliance, Legal, and Clinical Leadership to ensure operating models comply with applicable federal and state requirements, provider scope-of-practice requirements, patient consent and privacy regulations, prescribing requirements, and other healthcare regulatory standards.

Support development of state-specific operational requirements as the service line expands geographically.

    Billing & Revenue Cycle

Partner with Revenue Cycle and Compliance to ensure AL/IL services are structured and documented to support compliant and appropriate reimbursement.

Maintain working knowledge of professional billing, applicable places of service, E/M services, payer requirements, provider enrollment, documentation requirements, and billing considerations associated with physician and APP services in senior living environments.

Monitor revenue-cycle performance and work with appropriate teams to address documentation, coding, billing, or operational issues affecting reimbursement.

    New Location Assessment & Launch

Evaluate prospective AL/IL locations to determine operational and financial feasibility.

Assess the resident population, payer mix, existing provider relationships, clinical infrastructure, EHR availability, facility resources, expected visit volume, provider requirements, and financial opportunity.

Make recommendations regarding whether the organization should enter a location and which care-delivery model should be implemented.

    Provider Operations & Productivity

Develop provider staffing, scheduling, rounding, and productivity models appropriate to the AL/IL service line.

Monitor provider utilization, patient volumes, location performance, and operational efficiency while ensuring that productivity expectations remain consistent with quality, documentation, regulatory, and patient-care requirements.

    Facility Relationship Management

Develop strong relationships with senior living leadership and establish clear expectations regarding the organization's services, provider availability, communication, scheduling, medical record access, clinical escalation, patient consent, and responsibilities of both the practice and facility.

Serve as an operational point of escalation for significant facility concerns related to the AL/IL service line.

    Performance & Growth

Develop and monitor performance measures for the AL/IL service line, including patient volume, provider productivity, financial performance, documentation and billing performance, patient access, facility satisfaction, and quality indicators.

Use performance data to identify opportunities for expansion, workflow improvement, staffing changes, or modification of underperforming locations.

    Qualifications

    • Bachelor's degree in Healthcare Administration, Business Administration, Nursing, Public Health, or related field required; advanced degree preferred.
    • Minimum of 7 years of progressive healthcare operations experience.
