We’re partnering with a leading inpatient rehabilitation hospital in Yuma, Arizona to find an experienced Case Management Director to lead its growing Case Management department.
This is an excellent opportunity for an experienced RN Case Manager or Case Management leader who enjoys developing teams, collaborating across disciplines, and helping ensure patients receive the right care and a safe transition home.

Job Details
What You’ll Do:
Lead and support a Case Management team
Provide leadership, coaching, and guidance to Case Managers
Oversee Discharge Planning
Collaborate with physicians, nursing, therapy, social work, patients, families, and other healthcare partners
Monitor patient progression and help facilitate safe, appropriate discharges
Support Case Managers with complex patients and challenging cases
Maintain awareness of the department’s patient population and step in clinically when needed
Promote effective interdisciplinary communication and collaboration
Help support the department through continued hospital growth
What We’re Looking For
Active RN license
Bachelor’s degree in Nursing (BSN)
3+ years of hospital-based Case Management experience
Experience with Utilization Review and Discharge Planning
Demonstrated leadership or supervisory experience
Strong communication and interdisciplinary collaboration skills
