Community Coordinator

A community services organization is seeking a Community Coordinator in Harlem, New York, to support clients with care coordination, telehealth facilitation, and field-based outreach. This role focuses on connecting individuals to providers, appointments, and community resources while helping promote stability and access to services.

About the Opportunity:

• Shift: Day shift
• Schedule: Monday-Friday
• Hours: 9:00 AM - 5:00 PM
• Setting: Hybrid (primarily field-based and in-office, with some remote work)

Responsibilities:

• Coordinate client care by connecting individuals to providers, appointments, and community-based resources
• Facilitate telehealth visits and support clients in using virtual care tools
• Conduct field visits in homes, shelters, and community settings as needed
• Support appointment scheduling, transportation coordination, and visit reminders
• Track client services, follow up on referrals, and complete documentation and notes

Qualifications:

• Bachelor's degree in a related field such as mental health, social work, or a similar discipline
• Experience in the mental health service field
