Shift: Day Shift | 9:00 AM – 5:00 PM
Schedule: Monday–Friday | 40 Hours/Week

Position Overview

We are seeking a Client Navigator / Case Worker to support patients through screening, diagnosis, referrals, scheduling, and care coordination. The role requires strong healthcare scheduling experience, EPIC proficiency, and bilingual English/Spanish communication skills.

Required Qualifications

  • Bachelor's degree from an accredited college or university
  • Minimum 3 years of relevant experience
  • At least 2 years of healthcare or procedural scheduling experience
  • Proficiency with EPIC scheduling tools, including Snapboard and referral work queues
  • Strong organizational and attention-to-detail skills
  • Ability to manage high-volume scheduling
  • Strong communication and collaboration skills
  • Ability to prioritize competing demands and meet deadlines
  • Ability to provide coverage for other scheduling staff
  • Bilingual in English and Spanish

Preferred Qualifications

  • Experience with breast cancer navigation and care coordination
  • Experience with genetic testing referrals and insurance authorization processes

Responsibilities

  • Maintain patient databases, appointments, diagnostic workups, and care plans
  • Assist patients from initial screening through diagnosis
  • Coordinate internal and external referrals
  • Obtain medical records, mammograms, pathology results, and other documentation
  • Provide navigation support and assess barriers to care
  • Coordinate genetic testing referrals and verify insurance
  • Process financial hardship forms and communicate with testing companies
  • Refer patients with high-risk lesions or genetic risk to appropriate specialists
  • Maintain patient records, program data, meeting minutes, and compliance documentation
  • Facilitate financial clearance and counseling for preoperative breast surgery
  • Monitor tumor registry data for reporting and grant submissions
  • Communicate with clinicians and hospital departments to resolve workflow issues
  • Participate in community outreach and breast cancer education programs
  • Verify insurance coverage and authorizations
  • Monitor and triage referral work queues
  • Schedule patients as needed
  • Assist patients with MyChart enrollment
  • Report concerns and recommend program improvements

Special Requirements

  • Bilingual English and Spanish required
