As an advocate and QIDP-certified professional, the Case Manager will oversee a caseload of 12 individuals, helping them identify, plan, and achieve personal outcomes. The Case Manager ensures coordination across services (vocational, recreational, health, etc.) and promotes meaningful community engagement both on-campus and in the broader community.
