Position Purpose
This position is responsible for the overall coordination of client care for hospice patients. This includes the oversight of the planning, coordinating, organizing, and directing of the clients plan of care. Responsible for developing, maintaining, and managing the coordination of client services and medications necessary to meet client needs. In conjunction with the hospice interdisciplinary team and the hospice medical director, directs hospice episode of care, verifies ongoing hospice eligibility, and ensures compliance with federal, state, and The Joint Commission requirements and excellent nursing practice.
Nature and Scope
The Case Manager-Hospice is responsible to coordinate and provide hospice services to patients who are terminally ill and their families, complying with the agency policies and procedures. The incumbent must be highly motivated and demonstrate critical thinking skills. The Case Manager is responsible for managing the plan of care, coordinating additional hospice disciplines, providing education related to end of life, and conducting ongoing medication management. The ability to drive and be on-call are components of this position.
The Case Manager must demonstrate the knowledge and skills necessary to provide care, assess ongoing eligibility for hospice services, and provide end of life care. The incumbent provides care under the direction of the attending physician, the Hospice Administrator, and the Hospice Nurse Manager. Close collaboration with hospice medical providers and leadership is required on a daily basis.
This position does provide patient care.

Minimum Qualifications Requirements - Required and/or Preferred
- Name: Description
- Education:
- Must have working-level knowledge of the English language, including reading, writing, and speaking English.
- Graduate from an accredited School of Nursing; bachelors degree preferred.
- Appropriate education to obtain and maintain State of Nevada Registered Nurse licensure.
- Experience:
- At least one year of acute care experience and/or one year of home health or hospice preferred.
- Understanding of Hospice philosophy required.
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with patients and their families.
- License(s):
- Ability to obtain and maintain a State of Nevada Registered Nurse license.
- Valid State of Nevada or California drivers license and ability to pass Renown Healths Department of Motor Vehicle Report criteria.
- Must be able to transport self to patients home/facility.
- May be working in multiple locations in a workday.
- Required for this position:
- Fingerprints must be able to pass Nevada Division of Public and Behavioral Health (DBPH) background checks upon hire and every 5 years per State of Nevada Revised Statue (NRS 449.123) to remain in this position.
- Certification(s):
- Current BLS certification by The American Heart Association standards.
