About the Role


INSIPLE Coalition is hiring a Case Manager to walk alongside the participants our outreach team engages in Riverdale, Roseland, and West Pullman. You will help people at the highest risk of violence set goals, connect to services, and stay on a path toward stability, from first intake through each milestone. This role is for a patient, dependable advocate who earns trust and follows through.


Responsibilities


  • Manage a caseload of participants at the highest risk of violence involvement, from intake and needs assessment through goal completion
  • Develop an individualized service plan with each participant and review progress regularly
  • Connect participants to INSIPLE's workforce development programming and to employment, education, housing, behavioral health, legal, and reentry resources
  • Follow up on every referral to confirm the participant was served
  • Coordinate with Outreach Workers on referrals, warm handoffs, and participant safety concerns
  • Support participants and families in the aftermath of violence
  • Build and maintain a referral network of service providers and employers
  • Keep accurate, timely case notes, contact logs, and outcome data for grant compliance
  • Take part in team debriefs, trainings, and community events
Requirements


  • Minimum 2 years of experience in case management, social services, reentry support, or violence intervention
  • Working knowledge of community resources on Chicago's Far South Side
  • Ability to build trust with participants who have experienced trauma, incarceration, or violence
  • Strong organization and written documentation skills
  • Comfort with data entry and case management systems
  • Ability to work a flexible schedule, including some evenings and weekends
  • Valid driver's license and reliable transportation
  • Background check required; a prior record does not automatically disqualify a candidate
Preferred


  • Lived experience or deep credibility within Riverdale, Roseland, or West Pullman
  • Training in trauma-informed care or motivational interviewing
