Job Purpose

The Care Coordinator is responsible for overseeing and managing AHN member cases to ensure compliance with AHN requirements and provider care plans. This role serves as an advocate for members by identifying and resolving care-related issues while promoting continuity and quality of care. The Care Coordinator guides members through the healthcare system, providing education, support, and coordination to help them successfully navigate treatment plans and access appropriate services.

Key Responsibilities

  • Build and maintain effective, professional relationships with AHN partners and providers, ensuring consistent follow‑through and collaboration
  • Respond to all calls, emails, written correspondence, and client referrals within established service standards (within 48 hours)
  • Collect and analyze relevant information to estimate surgical costs and clearly communicate cost scenarios to members
  • Demonstrate working knowledge of CPT and ICD codes, medical billing processes, and insurance concepts to discuss balances and financial responsibilities with members
  • Maintain accurate, timely documentation by updating Salesforce records and other required systems
  • Adapt to department-specific schedules while managing multiple priorities efficiently in a fast-paced environment

Education & Experience Required

  • Bilingual (English/Spanish)- Required
  • High School Diploma required; AA Degree preferred
  • Minimum 2 years of experience working with socioeconomically diverse or disadvantaged populations
  • Proficiency in Microsoft Office and database systems
  • Community Health Worker (CHW) Certification (must complete within 90 days of hire)
  • Demonstrate strong critical thinking, problem‑solving, and communication skills
  • Maintain a valid driver's license and ability to operate an AHN company vehicle as needed
  • Must adhere to HIPAA regulation

Work Environment

  • The noise level in the work environment is usually moderate, and the employee usually works in a climate-controlled office environment
  • Ability to travel and drive for extended periods (up to four hours at a time)
  • Be able to sit at a computer for up to 6 – 8 hours per day
  • Be able to stand between 3 – 6 hours per day.
  • Be able to withstand hand/wrist deviation and repetition
  • Must be able to lift, carry up to 30 pounds with a fair consistency
