This is a great next‑step role for Direct Support Professionals who are ready to expand their impact through coordination, documentation, advocacy, and staff support-while still staying closely connected to the people served! We're seeking a detail‑oriented, people‑focused professional with a background in human services, health, or education-and hands‑on direct care experience-to oversee Direct Support Professionals, ensure 245D compliance, and support individuals in achieving their personal goals.
Position Details
Pay: $50,000-$60,000 salary (based on experience)
Schedule: Monday-Thursday, 8:00 AM-5:00 PM
(Office closed Fridays - salaried role)
Location: In‑office, Bloomington, MN
Type: Direct hire / permanent role with benefits

Key Responsibilities
Oversee, support, and evaluate daily service delivery to ensure clients progress toward personal goals
Ensure all documentation, Support Plans, forms, and procedures meet 245D regulatory standards
Complete after‑visit documentation, maintain accurate records, and prepare required reports
Communicate with case managers, counties, and care teams regarding follow‑up needs, service changes, and requests
Provide instruction, guidance, and competency observations for Direct Support Professionals
Serve as the primary liaison and advocate for individuals receiving services
Review and complete incident reports, emergency reports, and corrective action items
Add new services, verify documentation and training, and support ongoing staff development
Conduct required 245D home visits and ensure compliance with annual paperwork, training, and service requirements
Required Qualifications
Candidates must meet one of the following education/experience pathways:
Bachelor's degree in human services, health, or education + 1 year full‑time direct care experience
OR
Associate degree in a related field + 2 years full‑time direct care experience
OR
Postsecondary diploma in a related field + 3 years full‑time direct care experience
(Direct care experience may include work with individuals with disabilities or older adults.)
Preferred Qualifications
Experience working in a 245D‑licensed setting
Experience providing services in clients' homes
Strong organizational, time‑management, and interpersonal communication skills
