Duties Include: Oversee assigned Community Health Action Teams (CHATs), including coordinating team activities, supporting and collaborating with co-chairs, engaging community partners and volunteers, facilitating meetings, planning and attending community outreach and events, monitoring progress toward established objectives, and ensuring alignment with Franklin County Public Health's Community Health Improvement Plan, and local jurisdiction strategic priorities.
