Coordinates, assesses, implements, and revises the individualized plan of care, to include: Managing discomfort and providing symptom relief, providing specialized nursing skills related to palliative and end-of-life care, creating plans of care and clinical solutions to patient problems utilizing the nursing process and critical thinking skills, and determining scope and frequency of services needed based on acuity and patient/family needs. Must possess the ability to organize, coordinate, and direct staff members in patient care, ensure attention to detail, initiative and judgment in determining needs and nursing service programs for patients, and display tact in dealing with personnel, medical and administrative staff, and the public.
