Communicates effectively with physician offices, home health agencies, rehabilitation facilities, long term care facilities, and third party payers to identify goals to assure that patients receive the most appropriate, cost effective and efficient means of care. Facilitates timely documentation review with the Clinical Documentation Improvement (CDI) specialist, as necessary to ensure appropriate clinical documentation is available in the patient’s medical record to guide the care team in determining the expected length of stay.
