p>In addition to mentoring responsibilities, the LCSW Mentor provides direct clinical social work service, counseling, and discharge planning services to hospital patients and their families including biopsychosocial assessment and documentation of emotional, psychological, social, financial, environmental or follow-up referral needs. We are expanding access to care across our communities through two acute-care hospitals, including a Level II Trauma Center and a Level III NICU maternity unit, along with a cancer center, urgent care locations, and a growing network of primary and specialty care clinics.
