Coaching certification from a nationally recognized coaching organization is preferred.</p><p>Experience:</p><p>Health or Life coaching experience preferred.</p><p>I: Essential Duties/Responsibilities (Position):</p><ul><li>The Health Coach designs, with the client, individualized action strategies and personal accountability structures that lead to greater control of the patient’s disease (e.g. diabetes, high cholesterol, asthma, etc.) and more positive clinical outcomes.</li></ul><ul><li>Independently manages a caseload of clients and provides disease management and coaching services.</li></ul><ul><li>Tracks and monitors the progress of the clients over time and ensures progress toward a measurable goal.</li></ul><ul><li>Responsible for accurately recording pertinent patient data for the purposes of detailed record- keeping and tracking.</li></ul><ul><li>Implements a range of interventions/skills including counseling, motivational interviewing, coaching, education, referral, and advocacy to gain access to services and resources for patients</li></ul><p>This role offers a four (4) day work week as part of our commitment to employee wellness and work-life balance.</p>
