The tasks assigned to the SWA may include but are not limited to non-skilled, routine administrative activities necessary to ensure that participant needs are being addressed such as confirming the delivery of goods and services or start date of external services and supports ordered by the IDT as part of the Care Plan; assisting participants with Medicaid recertification; researching/identifying community-based agencies to address social determinants of health or other participant needs; other tasks as directed by the Social Services Supervisor. Goods, Services and Supports Fulfillment: The SWA may be assigned tasks that include contacting participants to confirm the delivery of critical DME, clinically indicated goods, services, or supplies and other interventions critical to supporting the participant in his/her setting.
