Here are just a few of the responsibilities: + Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence. + Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient. + Perform initial and comprehensive assessments on all patients in accordance with policies and procedures. + Continuous visits and interdisciplinary assessment of the patient. + Deliver quality care and completes clear, concise, and accurate clinical documentation. + Provide patient, caregiver and family counseling and education as indicated in the plan of care. + Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs. What it takes to be an RN Case Manager with Care Initiatives Hospice: + Active Iowa RN license. + Active CPR certification. + Hospice or palliative care experience. + Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred. + Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region. + Home access to the internet. + Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
