Direct Support Professional - Part Time

Responsibilities As the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.
