Provide support to individuals receiving services including: assistance with daily activities such as meal preparation and planning; medication administration; and personal care.

Maintain working relationship with agencies, community resources, and families to coordinate services for patient care; provide information on substance abuse treatment resources and services; make necessary referrals; facilitate follow-up to ensure appropriate services have been delivered; and maintain aftercare follow-up system. Assist patients in identifying and linking to appropriate resources for immediate needs and for the next levels of continuing care, safe and sober housing, and other recovery supports; assist patients with interviews and visits to prospective programs and providers; provide or arrange transportation to interviews.

The Active Treatment Coordinator performs moderately complex human services support work to include but not limited to: Organizing and coordinating leisure activities for individuals served which is age appropriate, based on their individualized interests, and is meaningful in reaching their goals.

Documentation includes plans of care, assessments, nursing notes, health summaries, referrals, acute care plans, change-of-status reviews, post-hospital assessments, medication refusal follow-up, post-injury reviews, treatment/SAM records, professional oversight logs, seizure and bowel movement records, transfer reports, ISP preparation, IHCP initiation, DSP instruction updates, training records, and tracking documents for weights, vitals, labs, consults, food intake, and follow-ups. Reviews and monitors health indicators, care plans, and clinical records, including vital signs, weight, nutrition, symptoms, medication refusals, injuries, seizures, bowel movements, labs, consults, hospital or infirmary status, IHCPs, acute care plans, DSP instructions, physician orders, MARs, treatment records, incident reports, preventive screenings, immunizations, EKGs, psychotropic medication monitoring, and other recurring health requirements.

Works collaboratively with the Special Services Leadership to design districtwide systems, supports and procedures to ensure the implementation of effective special education instructional services for students. Primary Purpose: The Special Services Coach is primarily responsible for providing training and classroom coaching to special education staff, general education staff, administrators, paraprofessionals, families, and other staff as appropriate.
