Responsibilities include assessing client needs, developing care plans, connecting individuals to community services, and advocating for their well-being. A Social Worker provides support and resources to individuals and families facing social, emotional, and health-related challenges.

Working Relationships: Coordinates activities of others (does not supervise) Works directly with patients and/or customers Works with internal customers via telephone or face to face interaction Works with external customers via telephone or face to face interaction Works with other healthcare professionals and staff Works frequently with individuals at Director level or above. Provides skilled Social Work services in the areas of comprehensive case management, discharge planning, continuing care services, advocacy, clinical social work services, including crisis intervention, and patient/family education utilizing individual and family modalities.

li>Perform individual and family support services, including but not limited to, teaching individuals and families self-advocacy skills; assisting with obtaining and adhering to medications and other prescribed treatments; building social skills in the community that will enhance quality of life, creating a crisis plan and/or psychiatric advanced directive, and identifying resources to support the client in attaining their highest level of health and functioning in their families and in the community. Health promotion will minimally consist of the following, but is not limited to, checking and monitoring client's vitals; providing health education specific to client's condition; developing self-management plans with the client; providing support for improving social networks and providing health promotion lifestyle interventions including substance use prevention; smoking prevention and cessation, obesity reduction and prevention; nutritional counseling; and increasing physical activity.

li>Works with clients to obtain and maintain eligibility for health and disability benefits, obtain and maintain affordable housing in a community of choice, arrange reliable transportation, locate and enroll in education or vocational programs, find and keep a job, locate needed services/resources and build a natural support system, and follow up to ensure services are received.

POSITION REQUIREMENTS:

Candidate/employee must meet the Oklahoma Department of Mental Health and Substance Abuse (ODMHSAS) criteria for Behavioral Health Case Management II (CM II) which requires:

  • a high school diploma or GED AND have a minimum 36 months of direct consumer care in the mental health and/or substance use field OR.
