The Behavioral Health Manager is a core member of an integrated care team that includes the patient's dialysis care team, nephrologists, and social workers, seeing patients in-person and virtually based on-site at the kidney care facility. Demonstrated experience supporting patients and families facing serious illness, end-of-life concerns, grief, and loss (e.g., long-term care, hospice, palliative care, oncology, or other high-acuity medical settings).
